This role involves assisting clients with daily activities, ensuring their safety, and promoting independence within a supportive environment. The ideal candidate will have a strong commitment to person-centered care and possess relevant experience in home and community care settings. Duties Assist individuals with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting Provide medication administration and monitor vital signs in accordance with care plans Support clients during meal preparation and feeding, ensuring nutritional needs are met Implement and follow personalized care plans tailored to each individual's needs Assist with mobility using equipment such as Hoyer lifts and ensure safe transfer techniques Observe and document patient conditions, including patient monitoring and patient observation Maintain a clean, safe, and comfortable environment through light housekeeping and cleaning tasks Support clients with memory care needs, including dementia or Alzheimer's care, ensuring safety and engagement Engage clients in social activities to promote mental stimulation and emotional well-being Comply with HIPAA regulations to maintain confidentiality of patient information Collaborate with healthcare professionals, families, and support teams to deliver comprehensive care Experience Prior experience working in assisted living, nursing homes, or home & community care settings preferred Experience working with individuals with developmental disabilities or autism is highly valued Knowledge of care for residents with dementia or Alzheimer's disease is advantageous Certification in CPR and First Aid required; additional certifications such as HIPAA training are a plus Experience with medication administration, vital signs monitoring, and use of mobility aids like Hoyer lifts is preferred Background in social work or long-term care enhances the ability to provide holistic support Candidates should demonstrate patience, empathy, strong observational skills, and the ability to work effectively in team environments This role offers an opportunity to make a meaningful difference in the lives of individuals requiring support while working in a compassionate and professional setting.
