Job Description

Transitional Support Floater Full-time, Pay Range:
$21.12 - $24.62 - Various Locations UCP Seguin of Greater Chicago is seeking compassionate, flexible, and dependable individuals to join our team as a Transitional Support Floater (TSF). This rewarding position provides direct support to adults with developmental and other disabilities across residential and day program settings. As a Transitional Support Floater, you will help individuals build independence, achieve personal goals, and actively participate in their communities while promoting dignity, inclusion, and self-determination.
Key Responsibilities:
High School Diploma or GED required. Must be at least 21 years of age. Six months of work or volunteer experience supporting individuals with developmental disabilities preferred. Ability to obtain and maintain required certifications, including: Medication Administration Training CPR and First Aid Certification Illinois DSP Training Curriculum Safety Care Behavior Intervention Training Strong communication, interpersonal, and problem-solving skills. Basic computer proficiency, including experience using computers, tablets, iPads, smartphones, and electronic documentation systems. Ability to work collaboratively across multiple programs and support teams. Physical ability to assist with personal care, transfers, mobility support, and behavioral interventions as needed.
Key Responsibilities:
As a Transitional Support Floater, you will support individuals in residential, day program, employment, recreational, and community settings. Responsibilities include assisting with personal care, meal preparation, housekeeping, medication assistance, transportation, community outings, and skill development. You will help participants build independent living, social, and community integration skills while implementing Individual Service Plans (ISPs) and maintaining accurate documentation. The role also includes collaborating with interdisciplinary teams, supporting medical appointments, participating in community-based activities, and ensuring the safety and well-being of those we serve. Schedule Transitional Support Floaters work flexible schedules based on the needs of assigned residential and day program locations. Team members provide support across multiple programs and may accompany individuals between Community Integrated Living Arrangements (CILAs) and Community Day Services (CDS) programs as needed.
