In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and well-being. Your expertise in patient observation, medication administration, and assisting with Activities of Daily Living (ADLs) will ensure our clients receive the highest quality care. This position offers an engaging environment where your caregiving skills help promote independence and comfort for those in need. Responsibilities Assist clients with ADLs such as bathing, dressing, grooming, toileting, and mobility to promote independence and dignity. Monitor and document patient vital signs, health status, and changes using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems accurately and promptly. Provide medication reminders and administer medications following medical guidelines while maintaining compliance with HIPAA regulations. Support clients with specialized needs including dementia care, Alzheimer's care, memory care, hospice support, and working with individuals with developmental disabilities or disabilities. Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively. Conduct patient observation to identify changes in condition or behavior and communicate findings to healthcare professionals promptly. Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe home environment for clients. Maintain detailed medical documentation and ensure all records are accurate and up-to-date in compliance with healthcare standards. Support clients' emotional well-being through compassionate communication and engagement during visits. Collaborate with healthcare teams to coordinate care plans tailored to each individual's needs in home or community settings. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated experience with patient care activities such as vital signs monitoring, medication administration, catheter care, and dementia or Alzheimer's support. Familiarity with EMR/EHR systems for documentation purposes. Experience working with individuals with developmental disabilities or long-term care needs is advantageous. Certification in CPR is required; additional certifications such as hospice care training or medical terminology knowledge are a plus. Proven ability to handle sensitive situations with professionalism while maintaining patient confidentiality under HIPAA guidelines. Strong communication skills coupled with a compassionate approach to caregiving are essential for success in this role.
