You will be part of a supportive team, providing essential care and coordination for patients and their families. Key Responsibilities

• Manage patient care in hospice settings
• Coordinate with healthcare teams
• Provide leadership and support to nursing staff

Schedule:

M-F ; 8a-5p (Start and end times vary from day to day

• expected to be available from 8am
• 5pm daily.

Oncalls/weekends:

o 1 to 2 Saturdays or Sunday per month

• either 8A to 5P or 5P to 8A (depends on staffing levels) o Additional stipends and compensation for fulfilling on call
• (2) holidays per year
• 1 summer & 1 winter; 3 floating holidays Travel
• Local travel required covering multiple counties.

Qualifications Required Hospice (1+ years) 1+ years of experience in ANY of the following: Home Health Care Epic Home Health Home Health Management (1+ years) High School/GED Valid Registered Nurse (RN) in Illinois (IL) ALL of the following valid licenses/certifications: American Heart Association Basic Life Support (AHA BLS) American Heart Association Advanced Cardiac Life Support (AHA ACLS) Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months?
