Home - Fortis About the Role We are seeking a compassionate and reliable Home Health Aide to support patients served by Select Home Health In this role, you will provide hands-on assistance in the home, helping patients with daily living activities while supporting their comfort, safety, and independence. No attendant care is provided (no housekeeping, errands, or companionship). Responsibilities Follow the instructions of the professional nurse/therapist in providing care. Provide assistance with personal care (bathing, oral care, and dressing) Maintain patient confidentiality and adheres to HIPAA requirements. Ambulation, positioning, and turning. Physical Requirements Ability to sit, stand, and drive for extended periods, with or without reasonable accommodation. You must be able to lift to 15 pounds occasionally, with or without reasonable accommodation. Regular use of phone, computer, and presentation tools.

Must be a certified Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in the state of Indiana. One year of direct patient experience in a hospital, facility, or home setting. Listed on the nurse aide registry with no finding against the aide relating to client abuse, neglect or misappropriation of client property. Willingness to travel between patient homes, visiting 6-8 clients per day, with visits lasting up to 1 hour per client.
