We serve a diverse population, including adults and children with intellectual and developmental disabilities, seniors, and individuals with complex care needs, across various care settings. Job Summary We are seeking a compassionate and dedicated Direct Support Professional to provide essential assistance and companionship to individuals with disabilities and complex care needs. The ideal candidate will possess a strong commitment to promoting independence, safety, and well-being through personalized support. This role involves working closely with clients to assist with daily activities, monitor health status, and foster a positive environment that encourages growth and community integration. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and document clients' health status, vital signs, and behavioral changes to ensure safety and well-being. Provide medication administration following prescribed protocols while maintaining compliance with HIPAA regulations. Support clients during meal preparation, cooking, and nutritional planning tailored to individual needs. Utilize patient observation skills to identify potential health concerns or behavioral issues promptly. Maintain a clean, safe, and organized environment by performing light housekeeping duties such as cleaning and sanitation. Operate medical equipment like Hoyer lifts or other assistive devices safely during transfers or mobility assistance. Collaborate with healthcare teams to develop and implement personalized care plans that address each client's unique needs. Experience Candidates should have prior experience working in group homes, assisted living facilities, or home health environments. Experience in dementia care, Alzheimer's care, memory care, or working with individuals with autism or developmental disabilities is highly valued. Knowledge of CPR, first aid, medication administration, and patient monitoring is essential. Applicants should demonstrate proficiency in handling heavy lifting tasks safely and possess strong communication skills for effective social work interactions. Prior long-term care or nursing home experience is advantageous; familiarity with HIPAA regulations is required to ensure confidentiality and compliance in all aspects of care delivery.
