This role focuses on assisting patients with daily living activities while promoting comfort, dignity, and independence. Key Responsibilities Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting Provide mobility assistance, transfers, and repositioning as needed Assist with meal preparation and light housekeeping related to patient care Monitor and report changes in patient condition to the supervising nurse or care coordinator Provide companionship and emotional support to patients Follow individualized care plans and agency policies Maintain a safe, clean, and supportive home environment for patients Document care provided accurately and in a timely manner Qualifications Valid Indiana HHA or CNA certification (required) Positive attitude and compassionate, patient-centered approach Prior home health experience preferred Reliable transportation and valid car insurance Physical examination within the last six (6) months Current TB skin test Ability to pass a background check Must pass a drug screen upon offer of employment We are an equal opportunity employer and does not discriminate on the basis of race, religion, color national origin, age, sex, gender, sexual orientation, disability or any other characteristic protected by law.
