  • Part-time, Full-time Overview Join our compassionate team as a Home Health Aide & Caregiver, where your dedication will make a meaningful difference in the lives of individuals needing support at home.
In this vital role, you will provide personalized care to promote independence, comfort, and well-being for clients across various settings. Whether assisting with daily activities or offering specialized care, your efforts will help clients maintain their dignity and quality of life. This position offers rewarding opportunities to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor and observe patients' conditions, accurately recording vital signs and changes in health status Administer medications following prescribed schedules and document medication administration in medical records or electronic health record (EHR) systems Support clients with specialized needs including dementia, Alzheimer's, memory care, and hospice care through compassionate engagement and tailored routines Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment Maintain strict adherence to HIPAA regulations to protect patient privacy and confidentiality during all interactions and documentation Utilize medical terminology appropriately when documenting patient care activities in electronic systems and medical records Requirements Proven experience in home health care, assisted living, nursing homes, or long-term care environments Knowledge of medical procedures including catheter care, vital signs measurement, CPR certification, and working with individuals with developmental disabilities or disabilities Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation Ability to work independently while managing multiple client needs effectively in a home or community setting Strong communication skills to collaborate with healthcare teams and family members while providing patient-centered care Valid CPR certification and willingness to obtain additional training such as hospice or dementia-specific certifications if needed Compassionate attitude combined with physical stamina to assist with mobility tasks and long shifts Join us if you're passionate about delivering exceptional patient care in a supportive environment that values your skills.
