Our family of companies includes Select Home Health & Hospice in Indiana and Valeo Home Health & Hospice in Utah, both locally operated agencies known for dependable, patient-focused care delivered in the home. About the Role We are seeking a compassionate and reliable Home Health Aide to support patients served by Fortis Health-affiliated agencies. In this role, you will provide hands-on assistance in the home, helping patients with daily living activities while supporting their comfort, safety, and independence. No attendant care is provided (no housekeeping, errands, or companionship). Responsibilities Follow the instructions of the professional nurse/therapist in providing care. Provide assistance with personal care (bathing, oral care, and dressing) Maintain patient confidentiality and adheres to HIPAA requirements. Ambulation, positioning, and turning. Graduate of an accredited High School or equivalent preferred. Ability to read, write and follow directions. Must have a valid drivers license, auto insurance, and your own personal reliable transportation. Must be a certified Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in the state which you are applying. One year of direct patient experience in a hospital, facility, or home setting. Listed on the nurse aide registry with no finding against the aide relating to client abuse, neglect or misappropriation of client property. Willingness to travel between patient homes, visiting 6-8 clients per day, with visits lasting up to 1 hour per client.
