This position offers a rewarding opportunity to make a meaningful difference in the lives of patients by assisting with daily activities, ensuring their safety, and promoting independence. Working Monday through Friday from 9 am to 5 pm, you will be an essential part of a supportive care environment that values professionalism, empathy, and excellence in patient care. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient health status, including vital signs such as blood pressure, pulse, and temperature Assist with Medication Reminders Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management Manage transfers safely to prevent injury Support patients during hospice or palliative care needs with compassion and respect Perform light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with HIPAA regulations Document all patient interactions accurately in medical records and ensure confidentiality at all times Collaborate with healthcare teams to coordinate patient care plans and communicate any changes or concerns effectively Engage with patients in home & community settings to promote well-being and independence while respecting their dignity Qualifications Proven experience working as a Home Health Aide or in long-term care settings such as nursing homes or assisted living facilities Strong knowledge of medical terminology, patient observation techniques, and caregiving best practices Familiarity with EMR/EHR systems for documentation purposes Certification in CPR preferred; additional certifications such as hospice care, dementia care, or medication administration are a plus Experience working with people with developmental disabilities or disabilities is highly valued Ability to perform physical tasks and assisting with mobility needs Excellent communication skills to interact effectively with patients, families, and healthcare professionals Knowledge of HIPAA regulations to ensure patient privacy and confidentiality at all times Ability to work independently while maintaining high standards of safety and professionalism Join us to provide compassionate home health care that truly makes a difference!
