Job Description

Job Summary Join our compassionate team as a Home Care Aide, where you will play a vital role in providing personalized, respectful, and high-quality care to individuals in their homes and community settings. Your energetic and dedicated approach will help enhance the lives of seniors, individuals with disabilities, and those recovering from illness or managing chronic conditions. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or wheelchairs. Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals. Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary, ensuring safety and comfort. Administer medications following care plans and HIPAA regulations to maintain patient safety and confidentiality. Support clients with memory care needs such as Alzheimer's or dementia by implementing specialized techniques and maintaining a safe environment. Perform light housekeeping tasks including cleaning, laundry, and maintaining a hygienic living space to promote health and well-being. Document care activities meticulously, update care plans regularly, and ensure compliance with all healthcare standards including HIPAA regulations. Provide emotional support through compassionate communication, engaging clients in social activities to promote mental health and social interaction. Collaborate with family members, healthcare providers, and social workers to deliver comprehensive home & community care tailored to each individual's needs. Use patient observation skills to detect early signs of health issues or behavioral changes for prompt intervention. Qualifications Prior experience in home health care, assisted living, nursing homes, group homes or long-term care environments is highly preferred. Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus. Experience working with individuals with developmental disabilities, autism, Alzheimer's disease or dementia is advantageous. Ability to lift heavy objects safely and perform physical tasks involved in patient transfer using equipment like Hoyer lifts or other assistive devices. Strong observational skills for monitoring vital signs and recognizing changes in patient condition. Knowledge of medication administration procedures and care planning processes. Compassionate attitude with excellent communication skills; ability to work independently while following detailed care plans.
