Job Description

We have an urgent need for a dependable Licensed Home Health Aide or CNA to care for one of our amazing patients in Lawrenceburg/Milan, Indiana. Duties:
The duties of a home health aide and or CNA include: the provision of hands-on personal care, the performance of simple procedures as an extension of therapy or nursing services, assistance in ambulation or exercises that are outlined in a RN-directed home health aide care plan. Maintaining records of personal health care activities. Maintain patient care areas in a clean, safe environment, adequate for the provision of care. Provides assistance in administering medications ordinarily self-administered (by the patient). Follow and document assignments in accordance with the home health aide care plan. Only document care actually given. Falsification of visit documents is grounds for immediate termination - falsification of visit documents is against the law and prosecution may ensue. Transfer patients in a safe manner, in accordance with standards of care. Home health aides are members of the interdisciplinary team. HHA must report changes in the patient's condition to RN or other appropriate skilled professional. Electronically document care given during each visit/shift. Report any shift time changes to the office. Call for authorization for extending a shift or adding a shift to assure that there is a payer source for the care. All hours/shifts must be assigned and approved by the Tendercare office to working. Failure to do so may lead to disciplinary action. Employees must pass a fit-to-work physical exam and be free from communicable disease .
