Job Description

Overview Join our dedicated healthcare team as a Home Health Aide/ Homemaker or companion, where your compassionate care and attention to detail will make a meaningful difference in the lives of patients receiving home and community-based care. In this vital role, you will provide personalized assistance to individuals with diverse needs, including seniors, those with disabilities, or patients requiring specialized dementia or hospice care. Your proactive approach and empathetic nature will help promote independence, comfort, and well-being in the familiar setting of their homes. Languages:
English, Russian ,but primarily Russian is required. (looking primarily for a female to care for other female clients) Responsibilities Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts or other assistive devices. Monitor patient health by observing vital signs, recognizing changes in condition, and accurately documenting medical information within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications and perform basic medical procedures such as catheter care, wound care, or hospice-related tasks following established protocols. Support patients with memory care needs by implementing specialized techniques for Alzheimer's and dementia care while maintaining a safe environment. Prepare nutritious meals and assist with light housekeeping duties including cleaning, laundry, and ensuring a hygienic living space to promote overall health. Maintain strict adherence to HIPAA regulations to protect patient privacy and confidentiality at all times. Collaborate with healthcare professionals to ensure comprehensive patient care plans are followed effectively. Qualifications Proven experience in home health care, assisted living facilities, nursing homes, or long-term care environments. Strong knowledge of medical terminology, medication administration, and medical documentation practices. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or dementia specialization are a plus. Experience working with individuals with developmental disabilities or disabilities in general is highly valued. Ability to operate EMR/EHR systems efficiently for accurate record keeping. Demonstrated skill in patient observation, vital signs measurement, and assisting with complex ADLs using appropriate equipment like Hoyer lifts or mobility aids. Compassionate attitude combined with excellent communication skills to work effectively with patients and their families. Valid driver's license and reliable transportation preferred for home visits.
