Join our compassionate team as a Home Health Aide, where your dedication and caregiving skills will make a meaningful difference in the lives of individuals needing support at home. In this vital role, you will provide personalized care to promote independence, comfort, and well-being for clients across diverse settings. Your energetic approach and attention to detail will ensure each person receives the highest quality of care, fostering a safe and nurturing environment. This position offers an engaging opportunity to work closely with patients, families, and healthcare teams, empowering those in your care to live their best lives every day. This job will require the staff to drive from one client's home to another in Boston and its neighboring towns such as Everett, Somerville, Hyde park, as well as Marshfield and neighboring towns such as Plymouth and Duxbury. Mileage will be reimbursed. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools. Monitor and observe patients' conditions by regularly checking vital signs and reporting changes promptly to healthcare professionals Provide medication reminders while maintaining strict adherence to HIPAA regulations for patient privacy. Support clients with specialized needs such as Alzheimer's care, dementia care, hospice care, and working with individuals with developmental disabilities or disabilities Document all patient interactions using Electronic Health Record systems. Prepare nutritious meals and assist with meal planning to promote health and well-being Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment in compliance with health standards. Collaborate effectively with nursing staff and other healthcare providers to ensure comprehensive patient care in home or community settings. Experience Prior experience in home health care or senior care environments is highly desirable Demonstrated ability to work with individuals with disabilities or chronic conditions such as Alzheimer's disease or dementia Familiarity with medical terminology, medication administration, catheter care, and vital signs monitoring Experience using EMR/EHR systems for medical documentation and record keeping Long-term care experience in nursing homes or assisted living facilities is a plus CPR certification required; additional certifications such as hospice care or hospice training are advantageous. Covid and TB vaccinations are required. Driver's license required.
