Duties Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility assistance using equipment like Hoyer lifts or transfer belts. Monitor patient health by taking vital signs, observing changes in behavior or condition, and reporting findings accurately to healthcare teams. Administer medications following prescribed care plans while maintaining compliance with HIPAA regulations to protect patient confidentiality. Prepare nutritious meals aligned with dietary restrictions and preferences, ensuring a clean and safe kitchen environment. Support residents in engaging in social activities, community outings, and recreational programs to promote socialization and mental stimulation. Maintain detailed documentation of care provided, patient observations, medication administration, and progress notes in accordance with facility policies. Follow safety protocols during heavy lifting or physical assistance tasks to prevent injury for both clients and staff. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Knowledge of patient care techniques including Alzheimer's care, dementia care, memory care, and autism support. Strong understanding of HIPAA regulations to ensure privacy and confidentiality of client information. Certified in First Aid and CPR; experience with emergency response procedures is essential. Ability to observe patients closely for changes in condition or behavior and document accurately for care planning purposes. Skilled in medication administration, vital signs monitoring, and implementing individualized care plans. Capable of performing light cleaning duties to maintain a safe and hygienic environment for residents. Physical stamina for heavy lifting tasks and assisting residents with mobility or transfers using appropriate equipment like Hoyer lifts or resident's . Compassionate communicator with excellent social work skills to foster trust and positive relationships with clients and families.
