You will be directly working with a female patient, following a one-on-one care plan in clients' home, this client is getting discharged from a rehab facility next week, desperately wants to come home, she's not allowed to go home until there is 24/7 assistance in place, You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. Requirements:
