In this vital role, you will provide essential support to clients by assisting with daily activities, monitoring health conditions, and ensuring their comfort and safety. Your dedication will help promote independence, dignity, and well-being for those in need of home and community care. This position offers a rewarding opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or wheelchairs Monitor patient health by taking vital signs, observing symptoms, and reporting changes promptly to healthcare professionals Administer medications accurately following medical instructions while maintaining detailed medical documentation in Electronic Health Record (EHR) systems Support clients with specialized needs including Alzheimer's care, dementia care, hospice support, catheter care, and working with individuals with developmental disabilities Prepare nutritious meals and assist with meal planning while adhering to dietary restrictions and medical guidelines Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment in compliance with HIPAA regulations Document all patient interactions thoroughly in medical records and ensure confidentiality of sensitive information Experience Prior experience in home health or long-term care settings is highly preferred Proven ability to work effectively with individuals with disabilities or cognitive impairments such as dementia or Alzheimer's disease Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation Knowledge of medical terminology, medication administration protocols, and vital signs monitoring Experience providing compassionate caregiving in assisted living or nursing home environments is advantageous Certification in CPR is required; additional certifications such as hospice care training or dementia care are a plus Demonstrated ability to handle medical equipment like Hoyer lifts safely and confidently Join us in delivering exceptional home health services that empower clients to live comfortably and independently.
