This role is often a strong fit for family members, trusted supports, and individuals selected by the participant or designated representative. Support Professionals work collaboratively with participants, families, and the Elevate Spectrum team to provide individualized, respectful, and community-focused support while honoring each participant's goals, preferences, routines, and Person-Centered Plan. Position Overview The Agency With Choice Support Professional provides direct support services that promote independence, dignity, safety, community inclusion, and meaningful participation in daily life. Services may vary depending on participant needs and authorized service types. This position may include Personal Supports, Community Development Services, workplace support, transportation assistance, and other participant-directed activities. Service Areas May Include Personal Supports Assistance with daily living activities, routines, personal care, safety, independence, and community participation Support with household tasks, meal preparation, medication reminders, and community access Assistance following individualized support strategies and routines Personal Supports Enhanced Higherlevel individualized supports based on participant needs and approved service plans Increased supervision, behavioral supports, or complex support strategies as directed by the participant's team and approved plans Community Development Services Skillbuilding activities focused on independence and community inclusion Support with volunteering, recreation, social engagement, transportation training, and community participation Assistance developing confidence, routines, communication, and selfadvocacy skills Ongoing Job Coaching Workplace support and assistance maintaining employment Support with workplace communication, routines, expectations, confidence, and job retention Assistance helping participants succeed in integrated employment settings Key Responsibilities Participant-Centered Support Provide respectful, personcentered support based on the participant's approved plan, goals, preferences, and routines Support dignity, choice, independence, safety, and meaningful community participation Assist participants with daily living tasks, community activities, workplace supports, or transportation as assigned Follow Service Implementation Plans, behavior support strategies, and participantdirected routines Collaboration and Communication Work collaboratively with participants, families, designated representatives, Program Managers, and the Elevate Spectrum team Maintain professional communication and respectful interactions at all times Participate in meetings, training, and team coordination activities as required Documentation and Compliance Complete required documentation, service notes, EVV requirements, incident reports, and timesheets accurately and on time Maintain confidentiality and professional boundaries Follow all DDA, Medicaid, EVV, and Elevate Spectrum compliance requirements Complete required onboarding, training, and annual renewals What Success Looks Like Participants feel respected, supported, and empowered in their daily lives Services are delivered consistently and according to approved plans Documentation and EVV requirements are completed accurately and on time Families and participants feel supported and maintain trusted support relationships Participants experience increased independence, inclusion, and meaningful engagement in their communities Qualifications High school diploma or equivalent preferred Must be at least 18 years old Ability to communicate respectfully and professionally with participants, families, and team members Ability to complete documentation and follow written support plans Reliable transportation may be required depending on participant needs Must complete required background checks, onboarding, and training requirements Prior experience in disability services, caregiving, community support, or selfdirected services is helpful but not required Preferred Experience Experience supporting individuals with intellectual or developmental disabilities Experience with selfdirected or familyguided support services Knowledge of
EVV, DDA
