In this vital role, you will provide personalized care in home and community settings, ensuring safety, comfort, and dignity for each patient. Your proactive approach and attention to detail will help foster independence and improve quality of life for those you serve. This paid position offers a dynamic environment where your skills in patient observation and assisting with activities of daily living (ADLs) are valued and essential.
Responsibilities:
Provide direct patient care by assisting with ADLs such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary. Monitor patients' health by taking vital signs, observing changes in condition, and reporting findings accurately in electronic health record (EHR) or electronic medical record (EMR) systems. Support patients with specialized needs including dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities. Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control standards are met. Document all patient interactions thoroughly in medical records while adhering to confidentiality standards set by HIPAA. Collaborate effectively with nursing staff and other healthcare providers to coordinate comprehensive patient care plans.
Experience:
Prior experience working in assisted living facilities, nursing homes, home health agencies, or community-based caregiving environments is highly preferred. Demonstrated ability to work with individuals with disabilities or chronic conditions such as dementia or Alzheimer's disease. Familiarity with medical terminology and documentation practices within EHR or EMR systems. Certification in CPR is required; additional certifications such as medication administration or hospice care are advantageous. Experience using medical equipment like Hoyer lifts or vital signs monitors is a plus. Proven track record of providing compassionate patient care while maintaining professionalism and confidentiality.
