We are looking for caregivers who are passionate about providing exceptional in-home care to diverse populations, including seniors, individuals with disabilities, and those requiring specialized memory or hospice care. As a member of our care team, you will provide personalized, one-on-one support that promotes dignity, independence, safety, and overall well-being. Responsibilities Provide compassionate, person-centered care to clients in their homes. Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, personal hygiene, and mobility assistance. Provide companionship and emotional support while encouraging independence and engagement in meaningful activities. Support clients living with dementia by using person-centered care techniques and maintaining a calm, safe, and structured environment. Assist with medication reminders or medication administration in accordance with your certification, state regulations, and agency policies. Monitor and report changes in clients' physical, emotional, or cognitive condition to the supervising nurse or office staff. Accurately complete daily documentation and maintain client records using agency-approved electronic documentation systems. Measure and document vital signs when required. Prepare nutritious meals and assist with feeding and hydration as needed. Perform light housekeeping duties, including laundry, changing bed linens, dishwashing, and maintaining a clean and safe home environment. Assist with transfers and mobility using gait belts, wheelchairs, Hoyer lifts, or other assistive equipment when appropriate. Transport and accompany clients to medical appointments, errands, and approved community activities. Maintain client confidentiality and comply with all HIPAA regulations and agency policies. Communicate professionally with clients, family members, supervisors, and other members of the healthcare team. Qualifications Previous experience caring for individuals with dementia or Alzheimer's disease is required. Current Home Health Aide (HHA), Certified Medication Technician (CMT), Certified Nursing Assistant (CNA), or Geriatric Nursing Assistant (GNA) certification preferred. Experience working in home health, assisted living, nursing homes, rehabilitation facilities, hospice, or long-term care settings is highly preferred. Strong understanding of dementia care, person-centered care, and memory care best practices. Knowledge of medical terminology and healthcare documentation. Experience using Electronic Health Records (EHR) or Electronic Medical Records (EMR) is preferred. Strong communication, organizational, and interpersonal skills. Ability to work independently while providing compassionate, high-quality care. Professional, dependable, and committed to delivering excellent client care. Requirements Current CPR certification required. Valid driver's license required. Reliable personal transportation required. Current automobile insurance required. Ability to safely transport clients to appointments, errands, and approved community activities. Ability to travel between client homes throughout the assigned service area. Ability to lift, transfer, push, and pull clients using proper body mechanics and assistive equipment.
