Job Description

Somerset Community Services, Inc.
Location:
Crisfield, MD. Part-time every other weekend. 11pm-9am Job purpose: To support individuals in the achievement of their goals while ensuring safety and monitoring of well-being.
Duties and responsibilities:
Assists individuals to connect in the community with social, educational, recreational and employment opportunities. Working one-on-one with each individual to assess his/her needs and tend to them accordingly with the goal of fostering a positive, encouraging environment. Provide support and accommodations for individuals working in the community. Take part in aiding and writing meaningful community-based outcomes that support each individual's person-centered plan. Assist individuals in using agency/public transportation. Maintain the rights, dignity and respect of all individuals served while promoting independence. Assist individuals in personal care, meal preparation and residential care. Maintain environment free of health and safety hazards. Assure safety and health of individuals receiving services. Maintain accurate records and generate reports as needed detailing activities, changes and progression of goals. Participate in all meetings and staff trainings. Supporting adults with developmental and physical disabilities Assist and support individuals to access and participate in the community in a meaningful way Administer medication under the guidance of nursing department Provides transportation as needed to and from facility-based locations Strong verbal and writing communication skills. Ensure an appropriate staff-to-client ratio at all times.
Qualifications:
High School Diploma One year experience assisting individuals with developmental disabilities preferred. Valid Driver's License. Meet agency vehicle insurance standards. Obtain training certifications in required timeframe. Ability to pass a background check. Must be able to use a variety of medical equipment such as wheel chairs, raised toilet seat etc. Must be able to work flexible hours including nights and weekends
