Job Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals needing assistance with daily activities and health management. In this vital role, you will provide personalized support to clients in their homes, ensuring their safety, comfort, and well-being. Your expertise in patient observation, medication administration, and home care procedures will help promote independence and improve quality of life for those you serve. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary. Monitor and document patient health status by taking vital signs and observing for any changes in condition or behavior. Administer medications accurately following medical documentation protocols while maintaining HIPAA compliance to protect patient privacy. Support clients with meal preparation and nutritional needs, ensuring dietary guidelines are followed. Perform light housekeeping duties including cleaning, laundry, and maintaining a safe, sanitary environment in accordance with home & community care standards. Use Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to update medical records and document care activities thoroughly. Communicate effectively with healthcare teams regarding patient progress or concerns related to dementia care, Alzheimer's care, or other specialized needs such as hospice or catheter care. Qualifications Proven experience in home health care, assisted living facilities, nursing homes, or long-term care environments. Strong knowledge of medical terminology and procedures related to patient observation, medication administration, and vital signs monitoring. Certified in CPR; additional certifications such as hospice care training or dementia/cognitive impairment training are preferred. Experience working with individuals with developmental disabilities or disabilities requiring specialized caregiving approaches. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping. Ability to work independently while following detailed care plans and adhering to HIPAA regulations for patient privacy. Compassionate attitude combined with excellent communication skills to foster trust and rapport with clients and their families. $18.00 - $22.00 per hour
