Full-Time / Part-Time Available Position Summary We are seeking qualified and dedicated individuals to provide professional in-home care for clients in the Midland area. This role focuses on the execution of individualized Plans of Service (POS) to ensure patient safety, promote independence, and facilitate active community engagement.
Primary Responsibilities Clinical Support & Monitoring:
Execute medication reminders, observe client health status, and provide detailed reporting on physical and emotional well-being. Plan of Service (POS)
Execution:
Follow established care protocols to meet specific health and safety goals for each consumer. Activities of Daily Living (ADLs): Provide professional assistance with bathing, dressing, personal hygiene, and meal preparation.
Community Advocacy:
Facilitate and transport clients to community adventures, including concerts, family functions, and medical appointments.
Operational Maintenance:
Manage routine household care, including laundry and personal space maintenance.
Qualifications Licensure:
Must possess a valid Driver's License and reliable transportation for community-based tasks.
Compliance:
Ability to pass a comprehensive background check and adhere to strict confidentiality standards.
Skills:
Strong communication, teamwork, and the ability to provide "active skill training" to help consumers handle challenging activities.
Experience:
Previous experience is preferred; however, we provide comprehensive on-the-job training for candidates who demonstrate professional potential.
Compensation & Schedule Pay:
$17.00 - $20.00 per hour (plus mileage reimbursement).
Shifts:
1st, 2nd, Respite, and Overnight shifts available.
Requirement:
Rotating weekend shifts (every other weekend). Must have a valid drivers license.
