Job Description

Job Summary We are seeking a compassionate and energetic Direct Support Professional to provide exceptional care and support to individuals with diverse needs, including developmental disabilities, dementia, autism, and other health conditions. In this vital role, you will empower residents to live fulfilling lives by assisting with daily activities, ensuring safety, and fostering a positive environment. Duties Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility to promote independence and comfort. Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare professionals. Administer medications following care plans and ensure compliance with HIPAA regulations to protect patient privacy. Prepare nutritious meals and assist with cooking while adhering to dietary restrictions and safety standards. Support residents during community outings or social activities to enhance social engagement and community integration. Operate medical equipment such as Hoyer lifts or other assistive devices safely during transfers or mobility support. Maintain clean and safe living environments by performing light cleaning duties and adhering to infection control protocols. Document care activities accurately in resident records, including progress notes, medication administration logs, and incident reports. Collaborate with team members to develop and implement personalized care plans tailored to each resident's needs. Provide emotional support and companionship to foster a caring atmosphere that promotes mental well-being. Requirements Prior experience working in assisted living facilities, nursing homes, or home & community care settings is highly preferred. Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus. Knowledge of dementia care, Alzheimer's care, memory care techniques, and working with individuals with developmental disabilities or autism is desirable. Ability to lift heavy objects safely (heavy lifting) and perform physical tasks required for patient transfers using equipment like Hoyer lifts. Strong observational skills for patient monitoring and patient observation duties. Excellent communication skills for collaborating with healthcare teams, residents, and families. Flexibility to work various shifts including evenings, weekends, or overnight hours as needed.
