In this vital role, you will provide personalized assistance to individuals in our assisted living community during the afternoon hours, ensuring their safety, comfort, and well-being. Your expertise in patient observation, and support with Activities of Daily Living (ADLs) will help promote independence and quality of life for those you serve. This position offers a rewarding opportunity to work closely with diverse populations, including seniors, those requiring specialized dementia or Alzheimer's care. Duties Monitor and observe patients closely, noting any changes in condition or behavior to ensure safety and prompt response. Assist residents with ADLs such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary.
Support residents with meals:
redirecting, feeding, ensuring dietary restrictions are followed. Record vital signs including blood pressure, pulse, temperature, and respiration rates using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for precise documentation. Charting observations in EHR and making sure ADLS are completed. provide assistance with escorting and transfers. (lifting required) Maintain a clean and safe environment by assisting with housekeeping tasks such as laundry and general cleaning in accordance with health standards. Qualifications Prior experience in home health care, nursing homes, assisted living or long-term care settings is highly preferred. Knowledge of medical terminology, medication administration procedures, and medical documentation is essential. Certification in CPR/AED is required; additional certifications such as hospice care or dementia training are a plus. Experience working with individuals with disabilities or behavioral challenges is advantageous. Ability to operate medical equipment like Hoyer lifts safely and effectively. Strong patient observation skills and the ability to respond swiftly to changing conditions. Excellent communication skills to collaborate effectively with residents, families, and healthcare teams while maintaining HIPAA standards.
