Summary As a Personal Care Assistant at Home Care Transitions, you will play a vital role in enhancing the quality of life for our clients. Located in southern Michigan, this position involves providing compassionate care and support to individuals in their homes, ensuring they receive the assistance they need to maintain independence. Responsibilities Assist clients with daily living activities such as bathing, grooming, and meal preparation. Provide companionship and emotional support to enhance clients' well-being. Monitor and document clients' health status and report any changes to the nursing staff. Help clients with mobility and transfers safely using proper techniques. Support clients in maintaining their personal hygiene and cleanliness of their living environment. Encourage social engagement and participation in community activities when possible. Requirements Previous experience in caregiving or a related field is preferred. Strong interpersonal skills with a compassionate approach to client care. Ability to lift and assist clients as needed while following safety protocols. Knowledge of home health practices and patient care standards is beneficial. Willingness to undergo background checks and training as required.
