Job Summary The Home Health Aide provides personal care and assistance to patients under the supervision of a registered nurse or therapist. You will help patients with daily living activities and support their independence in a safe home environment Responsibilities Provide personal care including bathing, grooming, and dressing Assist with light housekeeping and meal preparation Monitor patient safety and report changes in condition Document all care provided accurately in Kinnser (WellSky) Maintain professional communication with the nursing team and office staff Qualifications Active HHA or CNA certification (as required by the state) Experience using Kinnser (WellSky) — required At least 1 year of home health or caregiving experience preferred Compassionate, dependable, and able to work independently Reliable transportation and valid driver's license
