Job Description
~~June is a great month to start a new Career~~ Overview Trays Home Health Care LLC is seeking compassionate and dedicated Direct Support Professional(s) to provide high-quality assistance and care to individuals with diverse needs. The ideal candidate(s) will possess a strong commitment to promoting independence, safety, and well-being while fostering a respectful and supportive environment. This paid position offers an opportunity to make a meaningful difference in the lives of those served by delivering personalized care in various settings such as group homes, assisted living facilities, persons served residence, and home health environments. Responsibilities Assist individuals with Activities of Daily Living (ADLs) Observe for any changes in health or behavior, documenting findings accurately in compliance with HIPAA regulations. . Prepare nutritious meals tailored to individual dietary needs while maintaining cleanliness and safety standards in the kitchen areas. Support residents with memory care needs by implementing specialized strategies and ensuring a safe and comforting environment. Engage clients in social activities and community outings to promote socialization, independence, and overall well-being. Maintain a clean and safe environment by performing routine cleaning tasks and adhering to infection control procedures. Collaborate with healthcare teams, social workers, and family members to develop and update personalized care plans that reflect each individual's goals and needs. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly desirable. Knowledge of patient observation techniques, and HIPAA compliance. Strong interpersonal skills with a compassionate approach toward working with individuals with disabilities or chronic health conditions. Experience working with individuals with developmental disabilities or autism is advantageous. Basic cooking skills for meal preparation that meets nutritional guidelines for various health conditions. A valid driver's license and insured vehicle are required for community outings or transportation assistance. Must be able to work Monday-Friday between the hours of 9am-5pm. No overnights. No weekends.
