Little Canada, MN Job Details Part-time | Full-time $18 - $20 an hour 2 hours ago Benefits Paid training Mileage reimbursement Dental insurance Paid time off Opportunities for advancement Qualifications Passenger vehicle Driver's License Full Job Description Position Summary The Direct Support Professional (DSP) provides person-centered services to individuals receiving support under Minnesota 245D Home and Community-Based Services. The DSP assists individuals with daily living skills, community integration, health and safety monitoring, and implementation of individualized support plans. This role requires transporting individuals in the employee's personal vehicle to community activities, medical appointments, employment sites, and other scheduled services. Essential Job Responsibilities 1. Direct Care & Person-Centered Support Provide services according to each person's Coordinated Service and Support Plan (CSSP) and CSSP Addendum. Support individuals with Activities of Daily Living (ADLs), which may include: Meal preparation Bathing, Hygiene and grooming Medication administration (once trained and delegated) Household tasks Implement positive support strategies and behavior support plans (as applicable). Promote independence, dignity, and choice in all daily activities. 2. Community Integration & Transportation Transport individuals safely in your personal vehicle to: Medical and dental appointments Community activities Employment or volunteer sites Recreational outings Maintain valid driver's license, reliable vehicle and required vehicle insurance. Follow all Minnesota traffic laws and company transportation policies. Using Geoloation to document mileage and transportation services accurately. 3. Health & Safety Monitor for changes in health, mood, or behavior. Follow all emergency procedures and reporting requirements. Maintain client confidentiality (HIPAA compliance). Complete required documentation within required timelines. Participate in required 245D annual and ongoing training. 4. Documentation & Compliance Complete service notes that meet 245D and DHS documentation standards in Therap (electronic monitoring system) Report incidents according to DHS and company policy. Attend staff meetings and mandatory quarterly training sessions. Maintain compliance with background study requirements (NetStudy 2.0). Must regularly read and review all company correspondence by whatever means. 5. Professionalism and Respect All Staff must maintain professional attitudes at work and when referring to Harmony Homes Professionals, clients and Interdisciplinary team members outside of work. Professionalism requires strict adherence to data privacy practices and HIPAA regulations to safeguard all client information. Staff must refrain from gossip or sharing confidential details, and are prohibited from posting any work-related or client information on social media platforms. Maintaining a respectful, confidential, and ethical environment is essential for protecting both clients and the organization. Required Qualifications Must be at least 18 years old. High school diploma or GED preferred. Valid Minnesota driver's license. Reliable personal vehicle in good working condition. Current auto insurance meeting Minnesota minimum liability requirements (proof required). Ability to pass DHS background study. Ability to lift up to 50 pounds and assist with mobility as needed. Strong communication and documentation skills. Preferred Qualifications STABLE experience in 245D services, IHS, waiver services, or group home settings. (Same company). Experience supporting individuals with developmental disabilities, mental health needs, or behavioral challenges. Medication administration experience (or willingness to obtain training). CPR/First Aid Certifications. Vehicle & Driving Requirements Employees who use their personal vehicle must: Maintain active auto insurance (minimum state-required coverage or higher per company policy). Provide proof of insurance and vehicle registration annually. Maintain a clean driving record. Agree to motor vehicle record (MVR) checks. Follow company mileage reimbursement policies. Ensure vehicle is safe, clean, and properly maintained. Failure to maintain required driving credentials may result in removal from transportation duties or termination. Must use geolocation when driving for mileage payment. Work Schedule Full-time / Part-time / Overnight Awake shifts available. Weekend and holiday availability may be required. Flexible scheduling based on client needs. Compensation & Benefits Competitive hourly wage based on experience. Mileage reimbursement for approved transportation. Paid training. PTO (if applicable).
