Overview As a Direct Support Professional (DSP) with Careplus Healthcare Services, you will play a critical role in providing 245D services . Your work will involve supporting individuals with their daily activities, ensuring their physical, emotional, and social well-being, and fostering a sense of independence and community belonging. Key Responsibilities Assist residents with Activities of Daily Living (ADLs) , including bathing, dressing, grooming, and meal preparation. Provide 245D Basic and Intensive Services , including supervision, support, and skill-building activities. Monitor resident health and promptly report changes or concerns to appropriate medical staff. Administer medications as prescribed and ensure compliance with medication schedules. Support residents in participating in social activities and community outings to encourage engagement and interaction. Uphold resident rights by ensuring dignity, respect, and privacy in all care activities. Conduct patient observations and provide support or interventions as needed. Assist with transfers, mobility needs, and other tasks requiring heavy lifting , while maintaining safety protocols. Maintain a clean and safe environment by adhering to health and safety standards . Qualifications and Skills Experience in 245D services, nursing home, memory care, or similar settings is highly desirable. Understanding of person-centered care and the requirements of the 245D program. Familiarity with medication administration practices and patient monitoring techniques. Knowledge of resident rights and advocacy principles. Ability to assist with physical tasks , including heavy lifting, using proper techniques. Compassionate, patient, and empathetic approach to caregiving. Strong communication skills for effective interaction with residents, families, and healthcare teams.
