In this vital role, you will provide essential support to patients by assisting with daily activities, monitoring health status, and ensuring a safe, comfortable environment. Your dedication will help promote independence, dignity, and well-being for those under your care. This position offers an engaging opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care services. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote comfort and independence Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation in Electronic Health Record (EHR) systems Support medication administration following prescribed protocols while ensuring compliance with HIPAA regulations for patient privacy Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Operate medical equipment such as Hoyer lifts and catheters safely and effectively to prevent injury and ensure proper patient support Assist with mobility using assistive devices like wheelchairs or Hoyer lifts to promote safety during transfers and ambulation Perform light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a hygienic environment to support overall health and well-being Document all patient interactions accurately in medical records and communicate effectively with healthcare teams regarding patient status Support hospice care patients by providing comfort measures and emotional support during end-of-life stages Work collaboratively within home & community care settings to deliver personalized services that meet each patient's unique needs Qualifications Prior experience in home health care, assisted living facilities, nursing homes or long-term care environments is highly desirable Knowledge of medical terminology, vital signs measurement, medication administration, and infection control protocols Ability to work independently while following detailed care plans and medical instructions Strong communication skills to interact empathetically with patients and coordinate with healthcare professionals Experience working with individuals with developmental disabilities or cognitive impairments preferred Familiarity with EMR (Electronic Medical Record) or EHR systems for documentation purposes Valid driver's license and reliable transportation may be required for community-based visits Join us in delivering exceptional home health services that empower individuals to live healthier, happier lives!
