Job Description

Overview Join our compassionate team as a Home Health Aide (HHA), where you will play a vital role in providing personalized, high-quality care to individuals in their homes. This position offers an exciting opportunity to make a meaningful difference in the lives of patients by assisting with daily activities, managing medical needs, and fostering independence. As an HHA, you will work closely with clients, families, and healthcare professionals to deliver comprehensive home health services that promote comfort, safety, and well-being. Your dedication and expertise will contribute to creating a supportive environment where clients feel valued and cared for every day. Responsibilities Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and document patient observations such as vital signs, medication adherence, and overall health status using Electronic Health Record (EHR) systems. Assist with specialized care needs such as catheter care, Alzheimer's and dementia care, hospice support, and memory care. Support clients with meal preparation, feeding, light cleaning, laundry, and home safety to ensure a clean and comfortable environment. Utilize medical terminology accurately when recording medical documentation and communicating with healthcare teams. Operate assistive devices such as Hoyer lifts and perform safe transfer techniques to prevent injury. Administer medications following prescribed protocols while maintaining strict compliance with HIPAA regulations to protect patient privacy. Recognize signs of health deterioration or emergencies and respond promptly by notifying appropriate medical personnel or emergency services. Maintain detailed medical records and ensure compliance with all healthcare regulations including HIPAA standards. Engage in ongoing training on EMR/EHR systems, CPR certification, and updates on best caregiving practices to enhance service quality. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated experience caring for individuals with developmental disabilities, Alzheimer's disease, dementia, or those requiring hospice or memory care services. Knowledge of medical terminology, medication administration procedures, vital signs monitoring, and catheter care is essential. Proven ability to work effectively with seniors or individuals with disabilities in home or community settings. Experience utilizing EMR/EHR systems for accurate documentation of patient care activities. Long-term care experience that highlights your commitment to compassionate caregiving and patient safety. Ability to handle physically demanding tasks such as lifting patients using Hoyer lifts or assisting with mobility needs safely.
