This vital role offers an opportunity to provide personalized care in various settings, including assisted living facilities, group homes, and home health environments. Your efforts will promote independence, comfort, and well-being for those you serve, creating a positive impact every day. Responsibilities Assist residents or clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts. Monitor patient health by checking vital signs, observing changes in condition, and reporting findings to healthcare professionals promptly. Support medication administration and ensure adherence to care plans while maintaining HIPAA compliance to protect patient privacy. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking as required. Maintain a clean and safe environment by performing light housekeeping tasks, including cleaning resident areas and ensuring infection control protocols are followed. Document patient observations, care provided, and any incidents accurately in accordance with healthcare regulations. Engage with individuals with developmental disabilities, autism, dementia, or Alzheimer's disease through compassionate direct support and social interaction to enhance their quality of life. Requirements Prior experience working in assisted living, nursing homes, home health care, or group homes is highly preferred. Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus. Ability to lift heavy objects safely and perform physical tasks involved in patient handling. Strong observational skills for monitoring patient condition and recognizing signs of distress or deterioration. Experience working with individuals with disabilities or special needs, including dementia or memory care residents. Knowledge of medication administration procedures and care planning processes.
