In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and independence. Your energetic and proactive approach will help create a comfortable, safe environment for those in need of home health services. This position offers an opportunity to work closely with patients, families, and healthcare professionals to deliver exceptional care that promotes well-being and quality of life. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and transfer devices Monitor and record vital signs like blood pressure, pulse, temperature, and respiration to track health status Administer medications accurately following medical instructions and document medication administration in electronic health record (EHR) systems Observe patient conditions closely, noting any changes or concerns related to dementia, Alzheimer's disease, or other cognitive impairments Provide specialized care for individuals with developmental disabilities or those requiring memory care and hospice support Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment Maintain strict adherence to HIPAA regulations to protect patient confidentiality and ensure compliance with medical documentation standards Skills Strong knowledge of medical terminology and procedures related to home health care Experience working with seniors in assisted living or nursing home settings is highly desirable Proficiency in EMR (Electronic Medical Records) and EHR systems for accurate documentation Ability to perform patient observation diligently and respond promptly to changing needs Excellent caregiving skills with a focus on patient safety, comfort, and dignity CPR certification and training in catheter care, hospice care, dementia care, and Alzheimer's care are preferred Capable of working independently while collaborating effectively with healthcare teams and family members Physical stamina to assist with mobility tasks and perform home care activities efficiently This role is ideal for compassionate individuals eager to provide high-quality home health services.
