Day, Evening and Weekend Positions Job Classification:
Part Time and Full Time Positions Available Job Requirements:
High school diploma and/or GED and a valid driver's license are required with experience working with people who have developmental disabilities.
Purpose Statement:
Provide assistance with each individuals daily activities including medical needs, medication administration, hands-on personal care, medical appointments, implementing delegated nursing tasks and community involvement.
Reports to:
Program Manager Responsibilities:
Direct care of program individuals and implementation of ISP and behavior plans. Required to perform all duties in accordance with State of Missouri regulations and Restoring Hope's policies and procedures Complete and maintains all required State mandated trainings such as Medication Aide, CPR/First Aid, and all other trainings required by Restoring Hope Attend meetings and training sessions as required by Restoring Hope and complete annual trainings related to consumers' ISP and related plans Provides transportation to and from activities; if needed Assist individuals in the pursuit of recreational, social and community activities based on their preferences and individual plans. Promote participation and involvement in community outings. Works all assigned schedules, including but not limited to arriving and departing on time, requesting time off in advance (4 Hour notification for Call-Ins and/or Shift Changes), notifying your supervisor of changes to your assigned work schedule promptly. All Shift Changes must be approved by Program Manager. Ensures each individuals' rights are protected; treats individuals with dignity and respect Administers medications to individuals as assigned, observes individuals for any changes in behavior or health that may have resulted from mediation administration Ensures individuals are dressed neatly and appropriately and are always groomed ; assists and encourages individuals to care for immediate personal needs such as toileting, washing their hands, and eating Implements and carries out doctor's orders, administers medications as prescribed and perform all nurse delegations. Teaches individuals daily skills such as household chores, laundry, cleaning, and washing dishes, as appropriate according to their individual plans. For some individuals this may mean that staff are required to do all the required chores if the client is not physically capable of performing identified tasks. Observes individuals for evidence of injury or bruises and evaluates for changes in emotional and physical status Reports and noted problems to the RH Lead Staff and/or PM immediately and complete a written incident form Reports all medical related incidents to the RH Lead Staff and/or PM Acts as a representative for Restoring Hope in a professional manner when required, to families, service coordinators, State officials, community partners, local businesses, and employers Maintains resident confidentiality. Treats all individuals and coworkers with dignity, respect, and fairness; provides support to other team members when individuals are having behavioral issues Documents all program data, including but not limited to individual plan data, behavior plan data, community integration logs, assessments, individual funds request, mileage logs, maintenance request forms, and supply acquisition forms Documents individual's health (i.e. seizure charts, weight, bowel movements, fluid intake) Ensures all documentation is accurate and completed at the end of each shift Ensures MAR entries are completed daily Other duties may be assigned
