8a-4:30p Pay Range $21.85 - $32.80 Essential Functions Assists with discharge services including preparing discharge packets, providing current resource lists, and researching resources to meet identified discharge needs. Contributes to efficient and effective Departmental workflow through assigned responsibilities including: arranging transportation; maintaining clothing closet; assisting with guardianship application process; assisting ED staff; supporting computer hardware and software needs; completing payroll; managing supply/delivery of car seats; appointment scheduling; delivering equipment to patient's room and completing quality assurance audits. Ensures the delivery of required Medicare forms, including Medicare Important Message (IM) letters to all Medicare and Medicare Advantage inpatients within 2 days of patient discharge, MOON forms to all Medicare and Medicare Advantage patients in Observation status for more than 24 hours, and Condition Code 44 letters to Medicare and Medicare Advantage patients when the patient status has been changed from inpatient to observation status. Facilitates smooth exchange of necessary information by communicating with Care Coordination staff, other medical center staff and outside agencies. Completes ad hoc reports, filing, data entry and general clerical duties as needed; prioritizes and organizes work to ensure timely submission of needed reports at the request of leadership. Performs reception activities by responding to questions, assisting customers, and answering the telephone in accordance with departmental standards. As appropriate, escalates callers/issues to Departmental leadership. Completes all required staff education by the assigned due date. Documents all work activities on the day on the day of service provision. Education, Experience and Certifications High School Graduate; Associate degree in Secretarial Science or health related field preferred. 2 years office experience preferred.
