In this vital role, you will provide personalized assistance to promote independence, safety, and well-being in various settings such as homes, assisted living facilities, group homes, and nursing homes. Your energetic approach and attention to detail will help create a nurturing environment where clients feel valued and supported every day. Responsibilities Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by taking vital signs such as blood pressure, pulse, and temperature; observe and report any changes or concerns Administer medications accurately following care plans and HIPAA regulations to ensure safety and compliance Support residents with memory care needs related to Alzheimer's disease and dementia through engaging activities and attentive supervision Prepare nutritious meals tailored to individual dietary requirements while maintaining cleanliness and hygiene standards Operate assistive devices such as Hoyer lifts or wheelchairs safely during transfers to prevent injury Maintain detailed documentation of patient care activities, observations, and progress in accordance with healthcare protocols Collaborate with healthcare professionals, social workers, and family members to develop and implement personalized care plans Perform light housekeeping duties including cleaning client spaces to promote a safe and healthy environment Provide emotional support and companionship to foster a positive atmosphere for residents or clients receiving home & community care Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred Demonstrated knowledge of patient observation techniques, medication administration, and vital signs monitoring Hands-on experience with Alzheimer's care, dementia care, or working with individuals with developmental disabilities or autism is advantageous Certification in CPR and First Aid is required; additional certifications such as HIPAA training or hospice care experience are a plus Familiarity with care tools like Hoyer lifts or other mobility aids enhances your ability to deliver safe patient support Proven ability to handle heavy lifting safely while maintaining proper body mechanics during transfers or repositioning tasks Strong communication skills for working effectively with patients, families, healthcare teams, and social workers Join us in providing compassionate caregiving that transforms lives!
