Whether working in assisted living facilities, nursing homes, or home settings, you will be an essential part of a team committed to delivering exceptional patient care with professionalism and empathy. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and wheelchairs Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates Support medication administration following proper protocols while maintaining strict compliance with HIPAA regulations Observe patients closely for changes in condition or behavior, especially in dementia or Alzheimer's care settings Document medical records and care notes efficiently using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Assist with specialized care procedures such as catheter care, hospice support, and memory or dementia care routines Prepare nutritious meals and assist with meal planning while ensuring adherence to dietary restrictions Maintain a clean and safe environment by performing light housekeeping tasks like laundry and general cleaning to promote hygiene and comfort Requirements Valid certification as a Certified Nursing Assistant (CNA), Home Health Aide (HHA), or equivalent qualification required Prior experience in long-term care facilities, nursing homes, or home health environments preferred Knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation Ability to work with individuals with developmental disabilities or cognitive impairments such as Alzheimer's disease or dementia CPR certification is required; additional certifications such as hospice care training or medication administration are a plus Experience working with patients requiring assisted living or hospice care is highly desirable Strong patient observation skills combined with compassionate caregiving approach Ability to lift patients safely using equipment like Hoyer lifts and assist with transfers comfortably Join us to be part of a dedicated team that values your skills and passion for caregiving.
