Job Description

Overview Join our compassionate healthcare team as a Medication Aide / Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA) / Caregiver, where your dedication will make a meaningful difference in the lives of our residents and clients. In this vital role, you will provide personalized, respectful care to individuals across various settings, including assisted living communities, nursing homes, hospitals, and home care environments. Responsibilities Assist residents or clients with daily activities such as bathing, dressing, grooming, and mobility support to promote dignity and independence. Administer medications accurately according to prescribed protocols, ensuring proper documentation and adherence to HIPAA privacy regulations. Monitor vital signs including blood pressure, pulse, respiration, and temperature; record findings meticulously in electronic health records (EHR) systems like Epic or Cerner. Support patients with specialized needs such as tube feeding, catheter care, or wound management while maintaining strict infection control procedures. Observe and report changes in health status or behavior promptly to nursing staff or supervisors for timely intervention. Assist with medical documentation using EMR/EHR systems to ensure accurate record-keeping of patient care activities and medical histories. Provide compassionate support in memory care units specializing in Alzheimer's disease, dementia care, hospice care, or working with individuals with developmental disabilities or disabilities. Follow all safety protocols related to assisted living, hospital procedures, and home & community care environments to ensure a secure setting for residents and staff. Skills Proven experience working in assisted living facilities, nursing homes, hospitals, or home care environments with familiarity in senior care and hospice settings. Strong knowledge of physiology, anatomy, medical terminology, and nursing practices necessary for safe medication administration and patient monitoring. Skilled in operating EMR/EHR systems such as Epic or Cerner; capable of accurately documenting medical records and patient information. Excellent communication skills to effectively collaborate with healthcare teams and provide clear instructions to residents or clients. Ability to handle sensitive situations with empathy while maintaining professionalism; experience working with individuals with disabilities or cognitive impairments is highly valued. Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times. Competence in performing specialized tasks such as tube feeding, catheter care, vital signs measurement, and other clinical procedures requiring attention to detail. Demonstrated ability to adapt quickly in fast-paced environments while prioritizing safety and quality of care.
