In this vital role, you will provide personalized assistance, promote community engagement, and ensure safety and well-being through hands-on caregiving. Your energy and enthusiasm will make a meaningful difference in the lives of those you serve, fostering a nurturing environment that encourages growth, dignity, and respect. Duties Assist individuals with daily living activities such as bathing, dressing, grooming, and mobility using tools like Hoyer lifts when necessary. Support residents with medication administration, vital signs monitoring, and health-related documentation using electronic medical record (EMR) systems. Help with meal preparation, feeding, and maintaining nutritional needs aligned with individual care plans. Implement behavior management strategies to support residents with dementia or behavioral challenges associated with disabilities or Alzheimer's disease. Facilitate social activities, community outings, and recreational programs to promote engagement and socialization. Maintain a clean and safe environment by performing light housekeeping duties including cleaning resident areas and shared spaces. Document observations, incidents, and care provided accurately to ensure compliance with care standards and regulations. Skills Proven experience in assisted living facilities, nursing homes, or personal care homes working with seniors or individuals with developmental disabilities. Ability to perform vital signs measurement and medication administration confidently and accurately. Strong caregiving skills including assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support. Familiarity with EMR systems for documentation and reporting; experience with Hoyer lifts or other assistive devices is a plus. Excellent communication skills to collaborate effectively with team members, residents, families, and healthcare professionals. Valid driver's license for transportation duties related to community outings or appointments; experience in home & community care is desirable.
