In this vital role, you will assist clients with daily activities, monitor their health and well-being, and promote independence while ensuring safety and comfort. Your energetic approach and attention to detail will make a meaningful difference in the lives of those you serve. This paid position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those requiring specialized care such as dementia or autism support. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor patients' vital signs and observe for any changes in health status, reporting findings accurately to healthcare professionals Prepare nutritious meals tailored to individual dietary needs and assist with feeding when required Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces Support medication administration following care plans while adhering to HIPAA regulations to ensure confidentiality Provide companionship and emotional support to foster social engagement and reduce feelings of isolation Follow all safety protocols including CPR, first aid, manual handling techniques, and emergency procedures Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred Knowledge of patient observation techniques, dementia care, memory care, Alzheimer's care, and working with individuals with developmental disabilities or neurodevelopmental disorders Ability to perform manual handling tasks such as heavy lifting and the use of assistive devices like Hoyer lifts Familiarity with care plans, medication administration, vital signs monitoring, and patient monitoring practices Strong communication skills to effectively collaborate with healthcare teams, residents' families, and other caregivers Certification in CPR and First Aid is required; additional certifications related to hospice care or social work are a plus Demonstrated ability to work independently while maintaining a high standard of patient care in home & community settings Compassionate attitude with a commitment to supporting individuals with disabilities or geriatrics through personalized caregiving Join us in making a positive impact every day!
