Job Description
The Opportunity We are currently hiring a Community Support Worker (Direct Support Professional / DSP) to provide Nebraska Medicaid Community Support services (H2015) to individuals living with serious mental illness in the Omaha, Nebraska area. This position begins as part-time with an afternoon schedule , with a clear path to full-time as our program grows. This is a field-based role — you will work directly with clients in their homes and in the community, helping them build the practical skills and supports they need to thrive. What You Will Do As a Community Support Worker at Thrivion Mental Wellness, you will: Provide direct, in-home and community-based rehabilitation and skill-building support to individuals with serious and persistent mental illness (SPMI), including those with co-occurring developmental disabilities or substance use disorders Support clients in developing and maintaining activities of daily living including hygiene, meal planning, household management, and medication adherence Assist with budgeting, financial management, and navigation of community resources Accompany clients to medical, psychiatric, and community appointments to support engagement and follow-through Provide relapse prevention support and assist clients in identifying early warning signs and coping strategies Assist clients in accessing and coordinating services including mental health care, primary care, housing supports, transportation, and benefits navigation (Medicaid, SSI, SSDI, AABD) Implement individualized treatment plan goals developed in collaboration with the clinical supervisor and client Document services provided accurately and in compliance with Nebraska Medicaid and Joint Commission standards Participate in 30-day clinical supervision reviews and ongoing team communication Support crisis planning and assist clients in utilizing their crisis and relapse prevention plans Work as part of a collaborative, clinically supervised team to ensure coordinated, person-centered care Who We Are Looking For You are someone who shows up with consistency, compassion, and a genuine commitment to walking alongside someone on their recovery journey.
