Job Description

Job Summary We are seeking a compassionate and dedicated PRN Home Health Aide to join our team and provide exceptional in-home care to clients. This flexible position offers the opportunity to make a meaningful difference in patients' lives by assisting with daily activities, medical needs, and emotional support. The ideal candidate will be energetic, detail-oriented, and committed to delivering personalized care that promotes independence and well-being. As a PRN (pro re nata, or as-needed) aide, you will work closely with clients, families, and healthcare professionals to ensure safe and effective home health services. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report any changes promptly Support medication administration following prescribed guidelines while maintaining compliance with HIPAA regulations Provide specialized care for clients with Alzheimer's disease, dementia, or memory impairments through tailored activities and supervision Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions accurately and efficiently Assist with medical procedures such as catheter care, wound care, or hospice-related tasks under supervision Operate equipment like Hoyer lifts safely to transfer patients and prevent injury Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients Support clients with developmental disabilities or those in assisted living settings by fostering independence and engagement Maintain professional standards of patient care while adhering to all safety protocols and healthcare regulations Experience Prior experience in home health care or long-term care environments preferred; experience working with seniors or individuals with disabilities is highly valued Knowledge of medical terminology and basic nursing skills such as vital signs monitoring and medication administration required Experience with Alzheimer's care, dementia care, memory care or hospice settings is a plus Familiarity with EMR/EHR systems for documentation purposes preferred CPR certification required; additional certifications such as HIPAA training or caregiving credentials are advantageous Demonstrated ability to work independently while following detailed care plans and instructions Strong communication skills to effectively collaborate with healthcare teams, patients, and families Join us in delivering compassionate home health services that empower individuals to live comfortably and safely in their own homes.
