Job Description
Job Overview The Home Health Aide / Personal Care Assistant / Caregiver provides non-medical hands-on assistance and supportive services to patients in their place of residence in accordance with the patient's plan of care and under the supervision of a Registered Nurse. This role focuses on assisting patients with activities of daily living, providing companionship, supporting a safe home environment, and observing and reporting any changes in the patient's condition or environment to the supervising nurse. As an unlicensed direct care worker, this role does not perform delegated nursing tasks. Essential Duties & Responsibilities The following duties are illustrative of the essential functions of this position. They are not intended to be all-inclusive; the employee may be assigned additional related duties as needed. Personal Care & ADL Assistance Assist with bathing, dressing, grooming, oral care, toileting, and incontinence care. Assist with eating, including meal preparation and feeding when appropriate. Assist with mobility, transfers, and ambulation; assist with use of canes, walkers, and wheelchairs. Assist with medication reminders only — no medication administration. Homemaking & Environmental Support Perform light housekeeping tasks directly related to the patient's care, including changing bed linens, laundry, dishes, and tidying patient living areas. Plan and prepare simple meals consistent with the patient's prescribed diet. Assist with grocery shopping and errands as authorized in the plan of care. Companionship & Supervision Provide companionship and emotional support to the patient. Supervise patient safety, including fall prevention and wandering precautions where indicated. Encourage participation in approved activities and engagement. Observation, Documentation & Reporting Observe and report changes in the patient's physical condition, behavior, mood, or environment to the supervising RN promptly. Document services provided, time, and observations accurately on the agency's task sheet or electronic system. Report any concerns regarding patient safety, suspected abuse, or hazards immediately. Compliance & Patient Rights Adhere to the Home Care Clients' Bill of Rights (RSA 151:21-b) and HIPAA requirements. Maintain confidentiality of all patient and agency information. Comply with the patient's individualized plan of care; do not perform tasks outside assigned scope. Required Qualifications Demonstrated ability to read, write, and follow written and verbal instructions in English (or other agency-supported language). Valid NH driver's license, current auto insurance, and reliable transportation. Reliable, compassionate, patient, and respectful demeanor. Preferred Qualifications Successful completion of a Home Health Aide or Personal Care Assistant training program (75+ hours preferred). High school diploma or equivalent (GED) Prior experience providing in-home care, particularly with elderly or disabled patients. Experience with patients living with dementia or Alzheimer's disease.
