• Full Time and Part Time and PRN Heart to Heart Home Care is seeking professional, compassionate Direct Support Professionals who are enthusiastic about making a positive difference in the lives of our patients.

Requirements Driver's License and second form of ID Physical and PPD within the last year CPR/FA (if completed) Diploma or Equivalent Job Description Direct Support to consumers whether in an In-Home or Group Home setting. Provide support, transportation and assistance for each individual to enable her/him to complete daily routines in a safe, healthy, productive and dignified manner. This usually includes toileting, bathing, dressing, laundry, supervision of recreation.
