In this vital role, you will provide essential support to patients in their homes, helping them maintain independence, comfort, and dignity. Your expertise will contribute to a safe, nurturing environment while fostering trust and positive relationships with those you serve. This position offers the opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care. All roles are paid positions, emphasizing the value we place on your dedication and professionalism. Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to promote comfort and hygiene Monitor patient health by observing vital signs, assessing physical condition, and reporting changes promptly using Electronic Health Record (EHR) systems Support medication administration following medical documentation protocols and HIPAA regulations to ensure patient safety Provide specialized care for individuals with dementia or Alzheimer's disease through memory care techniques and patience-driven engagement Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and medical orders Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to support overall well-being Document all patient interactions accurately in medical records while maintaining confidentiality under HIPAA guidelines Collaborate with nursing staff and healthcare providers to coordinate comprehensive home health care plans Support patients during hospice care by providing compassionate companionship and symptom management Requirements Valid certification as a Home Health Aide or Certified Nursing Assistant (CNA) with current licensure as required by state law Proven experience in home health care, assisted living facilities, nursing homes or long-term care environments Strong knowledge of medical terminology, vital signs monitoring, medication administration, and infection control practices Experience working with individuals with developmental disabilities or dementia is highly preferred Ability to operate EMR (Electronic Medical Records) systems efficiently for documentation purposes CPR certification is required; additional certifications such as hospice care or Alzheimer's/dementia training are advantageous Excellent patient observation skills combined with compassionate caregiving abilities Physical stamina to perform caregiving tasks including lifting using Hoyer lifts and assisting with mobility needs Knowledge of HIPAA regulations to ensure patient privacy at all times Join us in delivering exceptional home health care that transforms lives!
