In this vital role, you will support patients by assisting with daily activities, monitoring health conditions, and ensuring their comfort and safety. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility Monitor and record vital signs including blood pressure, pulse, temperature, and respiration Support medication administration following proper procedures and documentation standards Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments Operate Electronic Health Record (EHR) systems to document patient information accurately and securely Support patients with catheter care, wound management, and hospice care needs Use assistive devices such as Hoyer lifts to ensure safe transfers and mobility assistance Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment Observe patient conditions closely, report changes promptly to healthcare professionals, and maintain HIPAA compliance for confidentiality Work with individuals with developmental disabilities or disabilities to promote independence and well-being Must speak Hindu and Punjabi . Experience Prior experience in home health or home care settings is highly preferred Experience working in assisted living or nursing home environments is advantageous Knowledge of medical terminology and medical documentation practices is essential Demonstrated ability to provide patient-centered care for seniors or individuals with disabilities Hands-on experience with EHR or EMR systems for accurate record keeping Certification in CPR is required; additional certifications such as hospice care or dementia care are a plus Long-term care experience and familiarity with hospice or memory care settings will strengthen your application Join our team to deliver compassionate, professional home health services that truly make a difference!
