Requirements:
Must be at least 18 years of age or older Driver's License valid in the state of NJ with a good driver's history Criminal Background Check Community Support Staff will not be listed on the Central Registry Skills Organizational skills and attention to detail Strong oral and written communication skills Good Computer Skills Creativity and problem-solving ability 1 year of experience working with individuals with developmental disabilities preferred. AA or BA preferred Essential Job Functions Develops and maintains an interactive relationship with each client that reflects values and principles held by Heart to Heart Home Care Participates in
ISP/ IDT
meetings. May assist in the development of the Individual Service Plan (ISP). Oversees the implementation of the ISP. Creates annual individualized goal sheets for each client's new ISP with manger and or director. Coordinates service provision within the community in conjunction with the Day Program Manager / Support Coordinator in order to meet the individual needs of individuals and to achieve each individual's ISP goals. Creates and managers housekeeping assignments for DSP staff (includes but does not limit: Laundry, dishes) Exhibits proficiency and implements all tasks needed to service the individuals appropriately including ISP objectives, upkeep of the work and activity areas, and other tasks as needed. Must complete all appropriate documentation properly. Plans and implements recreational activity plans for the individuals in the day program with adherence to the activity schedule. Identifies and implements strategies for maximizing the inclusion of clients in the life of the community. Administers medication and documents such in compliance with Heart to Heart Home Care's policies and procedures which conform to DDD guidelines. Orients new clients to basic safety practices in the day program and ensures that emergency drills are conducted as required. Establishes a warm, nurturing, and caring environment for the individuals in the day program and provides quality care for the individual throughout the week. Completes all DDD required trainings and continues to improve skills through trainings required by Heart to Heart Home Care. Transports individuals to community destinations as necessary or assigned in Heart to Heart's vehicle as well as supervises clients in the community during the outings. Must cooperate with Heart to Heart and Department of Human Services staff in any inspection or investigations. Follows through with additional responsibilities and tasks related to care and compliance as assigned by management or administration. Must be able to maintain client's physical hygiene and assist in client's toileting when needed.

Must be able to adhere to each client's specific dietary needs and properly feed clients per their dietary specifications. Must be physically able to lift 50lbs.
