The ideal candidate will possess a strong background in clinical social work, with expertise in patient assessment, case management, and behavioral health interventions. This role offers an opportunity to make a meaningful difference in patients' lives by delivering personalized care within their home and community settings. The successful applicant will demonstrate excellent communication skills, cultural sensitivity, and a commitment to maintaining the highest standards of patient confidentiality and compliance with healthcare regulations. Duties Conduct thorough patient assessments to determine individual needs, including mental health, behavioral health, and social support requirements. Develop and implement tailored care plans that incorporate evidence-based practices such as cognitive behavioral therapy, motivational interviewing, and crisis intervention strategies. Provide individual and group counseling sessions focused on mental health counseling, addiction counseling, dementia care, Alzheimer's care, PTSD care, and working with individuals with developmental disabilities or autism. Coordinate with healthcare teams to ensure seamless integration of services across outpatient, inpatient, hospice, assisted living, nursing home, and post-acute care settings. Manage medical documentation accurately within electronic medical record (EMR) systems while adhering to HIPAA regulations to protect patient privacy. Facilitate intake procedures and maintain detailed medical records using medical terminology and ICD coding (ICD-9/ICD-10) for diagnostic evaluation and billing purposes. Engage in data collection for clinical research projects related to behavioral health or specialized populations such as geriatrics or pediatrics. Provide crisis management support and case management services to ensure timely intervention and optimal patient outcomes. Collaborate with families and caregivers to educate them on disease processes such as dementia or autism spectrum disorder and support their involvement in treatment plans. Maintain ongoing professional development by staying current with healthcare regulations, CPT coding practices, and advances in social work methodologies. Experience Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program; licensure preferred. Proven experience working within home health environments or community-based care settings. Demonstrated expertise in child & family counseling, behavioral therapy, hospice care, or senior care is highly desirable. Prior experience with hospital or nursing home settings involving inpatient or outpatient services is advantageous. Familiarity with EMR systems, ICD coding (ICD-9/ICD-10), CPT coding, and medical documentation standards required. Experience working with individuals with disabilities—including developmental disabilities—and managing complex cases involving dementia or PTSD is preferred. Knowledge of HIPAA regulations, medical terminology, diagnostic evaluation techniques, and crisis intervention strategies is essential. Ability to handle sensitive situations with professionalism while demonstrating cultural competence and empathy.
