As a vital member of our caregiving team, you will deliver personalized assistance that promotes independence, safety, and comfort. Your role will involve working closely with patients, families, and healthcare professionals to ensure high-quality, respectful, and comprehensive care. This position offers an opportunity to make a meaningful difference in the lives of those you serve while developing your skills in a dynamic healthcare environment. Responsibilities Observe and monitor patients' physical and emotional well-being, accurately documenting changes or concerns using Electronic Health Record (EHR) systems. Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support. Medication reminder and medical instructions while maintaining compliance with HIPAA regulations for patient privacy. Vital signs measurement, and CPR when necessary, If outlines in Care Plan, ensuring safety at all times. Support patients with Alzheimer's disease, dementia, or memory care needs by implementing specialized techniques tailored to their condition. Prepare nutritious meals and assist with light housekeeping tasks such as cleaning and laundry to maintain a safe and comfortable environment. Document all care activities meticulously in medical records or documentation systems for accurate tracking of patient progress. Collaborate with healthcare teams during hospice or long-term care situations to deliver compassionate support aligned with care plans. Engage with clients in home and community settings, providing personalized care that respects individual preferences and cultural backgrounds. Experience Prior experience working as a Home Health Aide (HHA), In a Home Care environment is highly preferred. Demonstrated ability to work effectively with people who have cognitive impairments such as Alzheimer's or dementia. Strong knowledge of medical terminology, medication administration protocols, and basic clinical skills like vital signs measurement. Experience providing home health or community-based care that emphasizes patient safety, independence, and dignity. Long-term care experience involving hospice support. Certification in CPR is required; additional certifications such as hospice care training is a plus.
